Warheads will not explode on logic and manual R if they were spawned and attempted to explode in a protected sector, the only way to work around this is to destroy the ship and spawn in another one in a non-protected sector.

The warhead keeps its "Protected" state.
If an entity with warheads goes into a sector that has "protected", the warheads will refuse to explode even after leaving the sector. This effect is there till you unload/load the entity.
The warhead itself can still be destroyed with weaponry though, so it's just that the explosion effect doesn't trigger.

What version are you using? I can't reproduce it. I noticed something different (which looks like what you're seeing) in the latest dev build but that's related to something else.
I have tested it in the latest version (v0.198.485) and found it 100% reproducible, I am not quite sure why you can't reproduce it.

Yeah, can reproduce fine now, probably messed up and forgot that '+ protected' isn't persistent between shutdowns.
