I'm not sure how to reproduce unfortunately, but it does happen.
Managed to get it by opening the scanner history right after a TP but couldn't get it again afterwards.
Apparently it is caused by "Without being in an entity and pressing the scan history button causes a nullpointer and game crash." but I can't reproduce that either. Hopefully the logs will help.
